Site Lead
Reports To: Project Manager Hourly Wage/Salary: $XX hour

The Site Lead serves in a operational role to the Project Manager, in charge of carrying out all administrative and functions related to all positions, along with providing accurate and up-to-the-minute data and information to HC, Healthcare for the Homeless, vendors, consultants, and hotel management. This leadership position is responsible for the general operations and management of the site, including all service delivery areas and programs associated with the operation of the High-Risk Non-Congregate Site. The Site Lead performs the following duties personally or through subordinate managers and/or designees:

  • Work with on-site hotel staff to coordinate meals and emerging guest needs
  • Coordinate intakes and discharges and other partners to determine discharge timing and plan
  • Complete guest/occupancy census
  • Onboard new staff
  • Coordinate with on-site and community partners, including Health Care for the Homeless, shelter staff, etc.
  • Coordinate on-site security
  • Coordinate with staff runners
  • Provide direction/guidance for site support and runner staff
  • Lead staff huddles
  • Coordinate the inventory and ordering of supplies
  • Office supplies (pens, paper, clipboards, etc.)
  • Health supplies (gloves, masks, hand sanitizer, wipes, first aid, etc.)
  • Pet supplies (if applicable)
  • Problem-solving staff scheduling issues, guest needs, etc.
  • Support Project Manager with problem solving issues that cannot be resolved on-site or impact multiple sites such as food/laundry service or safety
  • Organize team meetings and staff training and identify overall needs of the sites

Qualifications: Enthusiastic, loyal, and results-driven. Must have a passion to see our hotel guests grow, expand, and succeed. Must have a passion to see MWBE enterprises grow, expand, and succeed. Bachelors or Masters Degree in healthcare, social sciences, nonprofit management, public administration. Minimum of five years; management & supervisory experience. Experience managing multiple staff members and an entire facility designed to help vulnerable populations. Able to work under pressure. Must know how to adhere to policy and rules in order to enforce policy and rules.

New Narrative Navigator (Site Support)
Reports To: Site Lead Hourly Wage/Salary: $XX hour

The New Narrative Navigator (Site Support Staff) serves in a facilitative role to identify guest resources, support guest needs, and work with site leads and runners to address issues. This leadership position is responsible for the general welfare and safety of residents on-site. The New Narrative Navigator performs the following duties personally or through designees:

  • Work with on-site hotel staff to coordinate meals and emerging guest needs
  • Nurture a safe, comfortable environment where guests are respected and valued for who and what they are
  • Lead meal distribution
  • Conduct attendance at meal times
  • Conduct room safety checks
  • Assist in maintaining a clean and healthy environment
  • Communicate with shelters about guests, as needed
  • Participate in shift transition and all team meetings
  • Coordinate with on-site Security as needed
  • Facilitate phone triage support—working with nurse practitioner or MD via phone for assessment, decision-making, and referral
  • Assist guests in navigating government, medical, and nonprofit/social services bureaucracies
  • Arrange for and coordinate resident transportation needs
  • Encourage each guest to develop a Personal Success Scenario
  • Facilitate games or group activities which are safe, safe, and, safe

Guest Services/Runner
Reports To: Site Lead Hourly Wage/Salary: $XX hour

The Guest Services/Runner Position serves in a delivery, errand, resource, and transportation, role to identify guest resources, support guest needs, and work with New Narrative Navigator site support staff to address issues. They serve as couriers for site and guest supplies and materials. Runners must own an insured vehicle and have a valid driver’s license. The Runner performs the following duties:

  • Assist site support in accessing supports and resources for guests
  • Provide language assistance, when needed/possible
  • Address guest questions and direct to appropriate staff
  • Assist with meals when needed
  • Assist in maintaining a clean and healthy environment
  • Go off-site to purchase supplies, materials, groceries, clothing, medications, mail, etc. (Debit cards will be provided)
  • Greet guests as they check in and direct them to appropriate staff
  • Provide general site support as needed when not running

RN & LPN Jobs

Qualifications: Active license with MN Board of Nursing & background checks are a must. Experience with quadriplegics, cerebral palsy patients, in-home care or care of special needs students will be an advantage but not mandatory.

PCA & Home Makers

We are hiring Personal Care Assistants and Homemakers for a variety of clients. PCA duties include bathing, feeding, toileting, grooming, dressing, light housekeeping, laundry, and other ADLs. Homemakers assist clients with housecleaning, laundry, meal planning/preparation and sometimes, grocery shopping. You may be combining PCA and Homemaker for some clients or either PCA or Homemaker for others. Flexible 30 – 40 hours weekly but may vary depending on your availability and work ethic.

Qualifications: CNA/NAR or High School Diploma/GED. You must take and pass the mandatory DHS online Training. If you already have your certificate, then kindly bring us a copy. Prior nursing home, assisted living facility, group home or home care experience is a plus but not compulsory.

Direct Support Staff, II.

Employees will professionally provide care and perform services for client as necessary to maintain personal comfort of client; maintain their personal health and hygiene in their place of residence; maintaining household records; shopping for groceries and preparing meals, grooming patients and handling basic logistics such as paying bills or scheduling appointments. Implements plan of care initiated by the registered nurse. Provides accurate and timely documentation consistent with the plan of care. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care. M-F, and some weekends. High School or GED and minimum of 2 years of Direct Care experience required.

Residential Home Coordinator

multiple positions Coordinate the operations of group homes and personnel; Program documentation at a high level, ensuring extensive daily documentation and all documentation is up to date. Prepares work schedule and reviews timecards; Conducts and attends staff meetings, providing input and ideas and new staff orientation. Maintains communication with family and friends of clients; provide direct care of clients when needed. Regular responsibility to be the On-Call Administrator. B.A or B.Sc minimum required.
